Proof of Claim Process

The prescribed Amwest in Liquidation Proof of Claim (POC) forms were mailed to all persons known or reasonably expected to have claims against Amwest Surety Insurance Company. The bar date (last day to file a timely Proof of Claim) was June 7, 2002.

Amwest will however continue to accept any and all claims filed on the prescribed Proof of Claim form, but they will be considered late. If you would like a POC form to file a claim, please contact Amwest at the toll free number or mailing address indicated on this Web Site (How to Reach Us). Please note that the POC forms will not be available from the Nebraska Department of Insurance.
